Strengthening Health Access Through Community Collaboration

The Archuleta Rural Health Network (RHN) brings together healthcare providers, public health leaders, and community organizations to strengthen coordination across the healthcare system serving Archuleta County.

Through collaboration, the Network works to ensure residents, especially those who are uninsured, underinsured, or navigating Medicaid and Marketplace coverage, have meaningful access to care, prevention services, and community health resources.

​Healthy Archuleta serves as the convener and backbone organization, helping partners coordinate efforts, share information, and align strategies that improve health outcomes in our rural community.

Why a Rural Health Network Matters

Rural communities face unique healthcare challenges, including:
  • Limited access to specialized healthcare services
  • Workforce shortages in healthcare and behavioral health
  • Insurance instability affecting access to preventive care
  • Transportation and geographic barriers to services
By working together, local organizations can better identify gaps, coordinate services, and strengthen the rural health system so that residents receive the care and support they need.

Our Shared Priorities

The Archuleta Rural Health Network aligns partners around two key priorities:

Improving Access to Care
The Network works to strengthen advocacy, education, and navigation supports that help residents connect with healthcare services.
This includes support for individuals who are:
  • Uninsured or underinsured
  • Navigating Medicaid eligibility or Marketplace coverage
  • Seeking preventive care and health resources
Community Health Workers and care navigators play a vital role in helping residents schedule appointments, understand insurance options, and access follow-up care.

Advancing Equity and Cultural Responsiveness

The Network is working to ensure healthcare services are accessible and responsive to all residents.
Current efforts include:
  • Expanding bilingual healthcare capacity
  • Strengthening interpretation services
  • Improving culturally responsive clinical practices
  • Ensuring materials and communication are accessible to diverse community members
These efforts help ensure that all residents feel welcome and supported when seeking care.

Strengthening the Rural Health Workforce

Workforce development is a key focus of the Network.
Community Health Workers (CHWs) are helping bridge gaps between healthcare providers and the community by supporting:
  • Health insurance navigation
  • Appointment coordination
  • Patient follow-up and referrals
  • Community outreach and education
Currently, there are a few credentialed Community Health Workers that serve Archuleta County, the majority of whom received training support through Healthy Archuleta’s efforts to expand Community Health Worker credentialing locally.

Monthly informal gatherings of healthcare advocates, navigators, Community Health Workers, and care coordinators further strengthen cross-agency collaboration and shared problem-solving.

Community Health Initiatives

The Rural Health Network supports community-wide efforts that connect residents with health services and information.

Archuleta Health Fair & Expo
The Network is committed to continuing and expanding the annual Archuleta Health Fair & Expo, which brings healthcare providers and community organizations together to offer:
  • Health screenings
  • Preventive services
  • Health education
  • Community resources

History of the Network

The Archuleta Rural Health Network entered a formal planning phase during 2024–2025, supported by funding from the Health Resources and Services Administration (HRSA).

During this period, partners assessed community needs, identified barriers to care, and developed a shared framework for rural health collaboration.  The Network was 
formally established in June 2025 and continues to serve as a cross-sector table dedicated to strengthening access to care, reducing barriers, and improving health outcomes across Archuleta County.
