Data Sources, ​​References and Resources, & Summaries and reports

Community-Level Data Sources
Information related to access and utilization of primary preventive health care in Archuleta County has been collected by various agencies in the community and presented below. The community participants that shared responses to the questions through these different datasources were conveniently drawn, which can limit the generalizability of the findings to the larger population.

Healthy Archuleta Data Sources
The following are the sources of information that have been collected in the community: 
  • Archuleta Food Summit Evaluation (2022), conducted in April 2022 with 104 evaluations completed. Evaluations were provided to participants of the 2022 Archuleta Regional Food Summit.
  • Archuleta Health Fair & Expo Evaluation Questionnaire, conducted during October 2023 with a total of 56 participants. The form was available in both English (53/56) and Spanish (3/56). Sampling - Convenience sampling with 56 respondents, the questionnaire was conducted via paper. The evaluation form was distributed during the lunch and presentation at the end of the Health Fair. It was observed that many of those who attended the Health Fair, especially those who came early on to access lab service and connect with healthcare providers, may not have stayed for lunch.
  • Archuleta Health Fair & Expo Assessment Questionnaire, conducted during October 2023 with a total of 113 participants. The demographics of this group were 62.6% (67/107) White, 32.7% (34/107) Hispanic, 2% (2/107) Asian, and 3.7% (4/107) indicated other races/ethnicities. Respondents reported their age as 3.9% (4/103) 18-25, 12.6% (13/103) 26-35, 31% (32/103) 36-45, 10.6% (11/103) 46-55, 26.2% (27/103) 56-64, and 15.5% (16/103) 65 and older. Regarding where participants lived, 35.5% (37/104) reported neighborhoods of PLPOA, 25.9% (27/104) downtown Pagosa Springs, 22.1% (23/104) other areas of Archuleta County, 8.6% (9/104) Aspen Springs, and 5.7% (6/104) outside of Archuleta County.   Sampling - Convenience sampling with 113 participants and vendors of the Archuleta Health Fair and Expo.
  • Archuleta Health Fair & Expo Registration Form, completed by 177 individuals who had registered for low-cost labs and/or the farm-to-table lunch. The demographics of the registrants were (7/160) 18-25 years old, (15/160) 26-35, (39/160) 36-45, (25/160) 46-55, (44/160) 56-64, and (30/160) 65+ years. For gender, (94/160) were female and (66/160) were male. Regarding race/ethnicity (91/166) reported White (Non-Hispanic), (65/166) reported Hispanic, (4/166) American Indian or Alaska Native, (2/166) reported Asian, and (4/166) reported other. Outreach for the Archuleta Health Fair & Expo was done primarily to those who likely did not have insurance through local businesses and outreach through the Spanish Language Equity Community Learning and Leadership.
  • Community Food Access Surveys, conducted in November and December 2019 at the Thanksgiving Turkey event and Toys for Tots event. 74 surveys were completed at the Thanksgiving Turkey event and 68 surveys were completed at the Toys for Tots event. Participatns at the two events were asked if they wanted to participate in the survey, participants at the Toys for Tots event (December 2019) were asked if they previsouly filled out the survey at the Thanksgiving Turkey event (November 2019). The majority of respondents lived in Pagosa Springs (92/135) followed by Aspen Springs (23/135) and other areas of Archuleta County (17/135).
  • Community Food Assessment Food Access Surveys, conducted in March 2020 at four food pantry locations (Community United Methodist Church, St Patrick’s Episcopal Church, Pope John Paul II Catholic Church, and Amazing Grace Community Church). Participants were invited to fill out two surveys, one that asked about their overall experience with food pantries in Archuleta County (survey 1) and one that asked about their experience at the food pantry where they completed the survey (survey 2). Survey 1 was completed by 136 participants and survey 2 was completed by 124 participants.
  • Community of Grower’s and Producer’s Survey, conducted in January - May 2020 with a total of 78 respondents. The survey was distributed electronically through the network of the Grower’s group and shared via partner listservs. The demographics of the respondents were 5% (4/76) ages 18-29, 14.5% 22.3% (11/76) 30-44, 22.3% (17/76) 45-59, 47.3% (36/76) 60-75, and 10.5% (8/76) 76 years or older.
  • Healthy Archuleta Commercial Kitchen Interviews, conducted during February 2023 with one location visited and additional data collection planned. Sampling - Targeted outreach to organizations with commercial kitchens. 
  • Healthy Archuleta Food Access Survey, conducted during December 2022 and January 2023 with a total of 115 participants. The demographics of this group were 54% Older Adults (65 years and older), 43% Adults (26-64 years old), and 4% Young Adults (18-25 years old); 54% White (Non-Hispanic), 35% Hispanic/Latin-x, Spanish, Cuban, Black/African American, Indigenous/Native American, Asian/Pacific Islander, and mixed, and 11% missing or other race/ethnicity.  Sampling - Convenience sampling of food pantry clients who were visiting the pantries the days the surveys were conducted. 
  • Healthy Archuleta Food Production Interviews, conducted during October and November 2022, with a total of 19 participants. The demographics of this group were 68% Adults (26-65 years old) and 32% Older Adults (65 years and older), and 100% of the respondents reported their race/ethnicity as White (Non-Hispanic). Sampling - Convenience and snowball sampling were used to invite interviewees. 
  • Healthy Archuleta Food Waste Prevention and Resource Recovery Interviews, conducted during May 2023 with representatives of the seven food pantries in Archuleta County. Sampling - Interviews and observations conducted at each of the seven food pantry locations.
  • Healthy Archuleta Growers and Producers Forum, held regularly since 2022 with the aim of connecting local growers and producers to learn from one another and increase local food production.
  • Healthy Archuleta Health Care Access and Utilization interviews (2022 Health Access Interviews), conducted during October and November 2022, with a total of 32 participants. The demographics of this group were 90% (29/32) Adults (26-64) and the remainder 10% (3/32) had representation from Older Adults (65+) and Young Adults (18-25); 50% (18/32) of respondents were White (Non-Hispanic), 41% (13/32) Hispanic/Latin-x and the remaining 9% (3/32) were Indigenous/Native American and Black/African American. Additionally, 37.5% (12/32) of respondents did not have health insurance and of the 62.5% (20/32) who had some form of health insurance, 60% (12/20) had private insurance (Blue Cross Blue Shield, Friday Health Plans, Rocky Mountain Health Plans, and United), 20% (4/20) had Medicare, and the remaining 20% (4/20) had other insurance types (Medicaid, cost-sharing program, or had military coverage).  Sampling - Convenience and snowball sampling were used to invite interviewees.
  • Healthy Archuleta Health Care Provider Survey, conducted during May-July 2022 with 10 health care agencies responding. The following definition of primary preventive care was given to providers in the survey: “Routine healthcare that includes screenings, check-ups, and patient counseling to prevent illness, disease, and other health problems as well as response to acute illnesses or to coordinate care among specialists.”  Sampling - Agency representatives completed the surveys; responses may have varied based on who at the organization completed the survey. 
  • Healthy Archuleta Spanish Food Access Survey, conducted in November 2022 with a total of 33 participants. The demographics of this group were 97% Adults (26-65 years old) and 3% youth (0-17 years old). All of the participants reported their race/ethnicity as Hispanic/Latin-x. Sampling - Convenience sampling consisted of community members who participated in a health insurance enrollment event. ​
  • OmniSalud Assessment Questionnaire, conducted during November 2023 with a total of 35 participants. The demographics fo this group were 97% (33/34) Hispanic and 3% (1/34) selecting other. For age, 6% (2/33) reported 18-25, 30.3% (10/33) 26-35, 39.3% (13/33) 36-45, and 24.2% (8/33) 46-55. Regarding gender, 35.3% (12/34) were male and 64.7% (22/34) were female. The majority reported living in downtown Pagosa Springs (66% or 22/33), 9% (3/33) in Aspen Springs, 18% (6/33) in the neighborhoods of PLPOA, and 6% (2/33) in other areas of Archuleta County. Additionally, 65.7% (23/35) of the respondents had OmniSalud health insurance during 2023, and 34.2% (12/35) had not previously had OmniSalud.  Sampling - Convenience sampling of 35 participants who attended OmniSalud insurance events hosted by Healthy Archuleta.
  • Pagosa Farmers Market Manager Interview, conducted in October 2023 with the Manager who has been with the market since 2007. The questions were developed and sent to the coordinator who provided written responses.
  • Seed to Supper Evaluation, conducted in March 2023 with the attendees on the last day of the 6-week Seed to Supper course in both English and Spanish. 30 evaluation forms were completed, with 24 completed in English and 6 completed in Spanish. The demographics of the respondents was  3.3% (1/30) 18-25 yrs, 70% (21/30) 26-65 years, 26.6% (8/30) 65+ years, 75.8% (22/29) White (Non-Hispanic), 20.6% (6/29) Hispanic/Latin-x, and 5% (1/20) other.

Partner Agency Data Sources
The following are the sources of information that have been collected by other local organizations: 
  • Aspire Community Survey, conducted during Spring of 2023 with 55 surveys completed. All respondents either had children or were pregnant,  10.91% reported being a single parent, 54.55% reported being married, 25.45% reported living with a partner, and 9.09% living with other family members (parent, grandparent, etc.).  Sampling: Convenience sample, primary outreach to parents of young children and clients of Aspire Medical Education and Services, which primarily provides prenatal and postpartum services.
  • Pagosa Springs Medical Center Quality Improvement Survey, conducted during August and September 2022 with 28 respondents. Sampling - Convenience sampling with 28 respondents, the survey was conducted via paper and the total number for each question was not included in the report.
  • San Juan Basin Public Health Community Survey, conducted during the Spring of 2023. There were 1,545 surveys completed across the two counties, with 1,118 in La Plata and  427 in Archuleta Counties. The demographics for the Archuleta respondents were 19% 20-44 years old, 48% 45-64 years old, and 34% 65 years and older. Regarding education, 1% did not complete high school, 5% had a high school diploma or equivalent, 18% had some college, 41% graduated from college, 7% had some graduate school, and 28% had completed graduate school. For gender, 71% reported women, 26% reported men, and 3% preferred not to respond. Sampling - convenience sample conducted in two counties - the primary mode of data collection was an online survey. ​
